By submitting an image for scanning or reproduction, you hereby represent that you own the copyright to such image or that you have permission from the rightful copyright owner to use the images submitted and hereby grant those rights to Pleasure Island and it's affiliates. |

| * We will turn over any photo(s) or disc(s) submitted containing child pornography to the proper law enforcement authorities and will assist them in the prosecution of such offenders. |

Model Release: I hereby certify that all images were taken of me, were taken with my permission and may legally and with my consent be scanned, copied or converted to any media by Pleasure Island Video or any of it's affiliates. I hereby grant sole ownership, license, copyright and a permanent irrevocable right to publish these images to Pleasure Island Video. No copies in any form or media may be sold or in any manner distributed or transferred without prior written consent from Pleasure Island Video. (We need photo copies of two forms of ID, showing date of birth.) |
